Our practice model is a Direct Pay or Concierge model meaning that we receive payments directly from our clients and do not bill or accept any type of medical insurance. We will be managing a smaller pool of patients. This allows us to treat each case with the time and attention needed and provide more thorough disease screening and prevention counseling. Without the time constraints of a traditional insurance-based model we can act as a patient advocate during any hospitalizations or emergency room visits by speaking directly to the providers caring for you. We can also facilitate sooner appointments with specialists to expedite your care.
Pricing and Discounts
Plans and Pricing:
Basic Plan $300/month
-Includes office visits with physician, up to a max of 24 per year
-Physician Cell Phone Access
-No diagnostic labs or imaging are included
Comprehensive Plan $350/month
-Includes office visits with physician, up to a max of 24 per year
-Physician Cell Phone Access
-Includes Calcium CT scan, Transvaginal US, Abdominal US
-Includes Wellness labs and ECG
Advanced Solutions Program $400/month
-Includes office visits with physician, up to a max of 24 per year
-Physician Cell Phone Access
-Four Home Visits and Four Mobile Lab Draws annually
-Everything included in the comprehensive plan
Discounts:
Annual contracts paid in full will receive a 10% discount. This discount can be combined with other discounts
Patients will receive a 10% discount for additional family members (Not valid for student plan)

An Annual Executive Physical that includes:
Panel of laboratory workup including complete blood counts, comprehensive kidney and liver testing, thyroid testing, hormone panels (if clinically appropriate), vitamin B12, folate, vitamin D, lipid screening, C-reactive protein and lipoprotein-a for additional cardiac risk stratification.
Annual ECG testing - an instantaneous look at cardiac conduction and rhythm
Coronary artery calcium scoring CT scan every 5 years - used to identify any plaque build up in the vessels of the heart and calculate cardiovascular risk
Total abdominal ultrasound annually - evaluates liver, gallbladder, kidneys, spleen, bladder, and low-resolution evaluation of the pancreas
Transvaginal ultrasound annually - evaluates the ovaries and uterus for patients who still possess said organs
All blood work and imaging will be organized and completed prior to the date of your 60-minute annual physical with your physician. This allows us to review the data together and make a comprehensive, collaborative plan to tailor your wellness goals for the year.
24/7 Access to your Doctors via phone
Additional visits throughout the year: we offer telehealth visits, travel counseling, acute care, and follow-up visits for chronic illnesses including mental health.
